CHD's Hawthorn Adult Day Health centers in Chicopee, Springfield, and West Springfield provide a variety of services and programs for seniors, including:
- assistance and training in activities of daily living
- educational and recreational programs
- nursing and psychological services
- physical, occupational and speech therapies
- nutritious meals and snacks
- transportation
Participants may choose from a variety of challenging and entertaining activities offered within the center or in the community. Participants may select community activities such as mall walking, bowling, visiting museums, casino trips, coffee klatch groups and/or in house activities—discussion groups, music groups or live entertainment.
A comprehensive clinical team, along with each participant, work together to create a plan of care designed to enhance the individual's level of function.
Referrals
Referrals can be made to our Adult Day Health program by family members, discharge planners, community agencies or health care professionals. Interested parties may call us at 413-205-2944 to obtain further information and arrange for an interview.
Transportation
All our Adult Day Health centers have van service available to provide transportation to and from the centers.
Hours
Our Adult Day Health center programs operate 8:30 a.m. to 4 p.m., Monday through Friday, except holidays.
